Kaplansky Foot and Ankle Centers welcome new patients and accept almost every local and national insurance plan.

No foot or ankle pain is normal and should be addressed right away. Don’t wait until it gets worse. The sooner we can treat your problem the faster we can manage and reduce your symptoms.

We understand going to the doctor can cause some anxiety. Our entire staff works hard to make your visit as comfortable as possible. We take the time to listen to your concerns and answer any questions that you may have.

With access to the latest technology in foot and ankle care, many times you can avoid more invasive procedures with our advanced up-to-date treatment options.

For your convenience, please click on the links below, print and fill out the new patient intake form. Please bring forms with you to your initial appointment as this will result in less waiting room time as well as more accurate information for the doctor.

Please also bring your current insurance card, a photo ID and any previous medical records pertaining to your condition. This includes but is not limited to any imaging such as MRIs, X-rays, or CT scans.
